Chronic Disease Management (Active Aging) Program

Offering a variety of structured and therapeutic exercise programs throughout Windsor-Essex County. Clients who take part in our programs live with one or more chronic health conditions such as Arthritis, Stroke, Heart disease, Diabetes, Respiratory Disease, COPD, Parkinson's, MS, Obesity, and other Chronic Illness. We offer programs including education, exercise,self management, support and Community connection. Within our structured programs we work towards improving cardio respiratory endurance, muscular strength, balance and flexibility to ensure clients are meeting their goals and maintaining their independence.

Our interdisciplinary team assesses each client individually to ensure we understand their goals, medical history, functional fitness levels so that each client is placed in an exercise program that will best suit their needs.

Our team of  Certified professionals include Nursing, Physiotherapist, Physiotherapist Assistants, Fitness Instructors, Rehab Assistants, Recreational Therapist,  Personal Support Workers, Occupational Therapist, Speech Language Pathologist, Social Worker, Respiratory Therapist, Dietician and Medical Secretary.

Referrals from a Physician is not required.  Self referrals are welcome.

Program placement and referrals for services will be determined at time of assessment.

Our Team Goal is to assist clients in staying healthy, active, safe, and living at home. 

As a Community Health Centre, our mission is to support the health and wellness of our vulnerable population in everything we do.

The Erie St. Clair LHIN largely funds our programs and services allowing programs to be supported and serviced at no cost to the clients.
