Regional Initiatives - Chatham Kent

Chatham-Kent Community Health Centre - Home Safety and Grab Bar Program - This program targets seniors aged 65 years+, who are at risk for falls. The program covers the cost of grab bars and their installation for people who cannot afford to do so. This program is for both CHC clients and non-CHC clients. Non-CHC clients are required to submit  a referral form to the CHC OT. Clients may also make a self-referral for a home safety assessment, following which grab bars would be installed if needed. VON - Exercise Program - As part of Ontario’s commitment to help more seniors stay healthy and active, the Erie St. Clair Local Health Integration Network (LHIN) has chosen the ESC VON to provide group exercise and falls prevention classes. Click here for more details on the VON Exercise Program. 

Age Friendly Community - Age Friendly planning is a global initiative of the World Health Organization (WHO), designed to encourage communities that are supportive of active aging by creating environments that are more inclusive, respectful and accessible. Age Friendly Chatham-Kent is lead by the Senior Advisory Committee, and with the assistance of community partners and leaders, is responsible for drafting an Action Plan.
