Chronic Disease Self-Management

The Windsor–Essex Community Health Centre (weCHC) helps support both individuals and health care providers with ongoing education and  mentorship by offering self-management workshops throughout the  Erie St. Clair LHIN.

Self-management refers to the skills and confidence to manage symptoms, treatment, physical, emotional, psychological, financial, and lifestyle changes that comes with living with a chronic condition.

Workshops for Individuals

1.    Master Your Health

A series of 6 weekly 2.5 hour interactive group workshops.

Clients who have completed the program experience high improvements in exercise, mental wellness, general health, distress, fatigue, disability, and activities of daily living in addition to fewer outpatient visits and hospitalizations.

2.    Powerful Tools for Caregivers

A 6-week series of 1.5 hour interactive group workshops that help  caregivers develop a wealth of self-care tools.  Caregivers learn how to reduce stress, take care of themselves, and increase their ability to make tough decisions.

3.     Diabetes, Healthy Feet & You

A 2.5 hour interactive workshop providing support and education on foot care.   Participants learn how to assess the impact of diabetes on the health of their feet, identify factors that can lead to foot ulcers and learn daily care for their feet.

4.    Getting the Most from your Healthcare Appointment

A 1.5 hour interactive workshop focusing on what to do before, during and after an appointment and use action planning to improve health.  This workshop prepares and empowers individuals to communicate more effectively with their healthcare team.


Workshops for Healthcare Professionals

1.    Choices & Changes: Motivating Healthy Behaviour

A 5 hour interactive education workshop to build effective and  evidence-based communication strategies and motivational interviewing skills that can be utilized in the clinical setting. 

2.    The Empathy Effect

A 4 hour skills development workshop that engages participants with activities on the six essential communication techniques identified in the scientific literature. These specific strategies used by health professionals provides care that builds trust with patients.
