weCHC on Wheels provides an entry point to healthcare services for people who often face barriers accessing traditional health care services. The program works in close partnership with local agencies to ensure it is meeting the needs of the surrounding community.
Services that are provided by physicians/nurse practitioners, nurses, and social workers/support staff consist of, but are not limited to:
- Diagnosis and Treatment of Conditions
- Case Management and Brief Intervention Services
- Referrals to healthcare providers, specialists and community supports
Accessing health and social supports is the key to helping individuals manage their well-being and prevent further illness. There are many factors that get in the way of accessing services. Some individuals will not attend traditional health care locations. Others are physically unable to access these services. Accessing health care can be challenging for those who do not have the means or have competing priorities such as lack of housing.
weCHC on Wheels is an important link with health care professionals and outreach workers. We can provide immediate care, and provide connections to ongoing support at the Windsor Essex Community Health Centre and community partners.
