Coordinated & Collaborative Care

Black Health offers culturally responsive and affirming counselling, education, caregiver support, and community connections for Black families. We provide a safe and welcoming space for Black families to talk about their physical and mental health. Our goal is to support and uplift every individual’s cultural identity, with a healing-centred, anti-oppressive, and anti-racist approach that emphasizes strengths.

Our goal is to connect families with the resources they need to thrive and with providers who value and understand their unique cultural experience. This includes connections to primary care, pre- and postnatal care, employment, school, or social supports.
