Diabetes Wellness
The weCHC – Diabetes Wellness team comprises registered nurses and registered dietitians, most of whom are certified diabetes educators. Diabetes Wellness is a community-based diabetes education program.
Our Clients are:
- Adults aged 18 years or older and diagnosed with prediabetes, Type 1 or Type 2 Diabetes.
- Adults aged 18 years or older and at risk of developing Diabetes.
- The caregiver or support person for someone living with Diabetes.
No referral is required.
